ABSTRACT

Case management is a flexible, systematic, holistic new care model which can integrate the episodic therapy of cancer patients. Case management applied the process of assess, plan, implementation, evaluation and feedback to the process of case management, conduct the path management from diagnosis to follow-up. This paper introduces the job descriptions and competence of case managers, the development of cancer case management domestic and overseas; so as to offer reference for the domestic implementation of cancer case management. Consequently realize the comprehensive rehabilitation of physical, psychological, social and spiritual function for cancer patients.

ABSTRACT

The magnamosis device for stage-one repair of the rectovaginal tistula consists of two arc magnets. Drawing the interrupting thread along the fistula margin via the vaginal side, and pulling the string to arrange the magnets at the fistula base along the long axis of the vagina, we made the magnamosis device automatically clipped to seal the fistula. After removing the threads we kept the device for 2-4 weeks till the natural detachment of it when the compressed tissue in between healed after vascular necrosis. This device utilizing the unique ability of magnamosis to fulfill anastomosis under inflammatory infected state reduces the current high relapse rate and colostomy drawbacks of the conventional rectovaginal neoplasty.

ABSTRACT

Objective To sum up effective measures to prevent complications of peripherally inserted central catheter (PICC) by applying evidence-based nursing in patients after PICC insertion.Methods From January 2011 to January 2013,1490 patients inserted with PICC in Hunan Provincial Tumor Hospital were selected by convenience sampling,whom were divided into control group (n =758) and observation group (n =732) with random number table.The control group received routine care,while the observation group received evidence-based nursing.Nursing program for the observation group was formed based on literature regarding the causes of and solutions for phlebitis,malposition of PICC,infection of insertion sites,and hemorrhage of insertion sites.Results The incidences of postoperative phlebitis,malposition of PICC,infection of insertion sites,and hemorrhage at insertion sites in the observation group were significantly lower than those in the control group (3.14% vs.8.97%,P=0.023;4.23% vs.11.74%,P=0.021;3.96% vs.8.44%,P=0.001;4.92%vs.12.66%,P =0.011).Conclusions The application of evidence-based nursing in patients with PICC insertion could effectively reduce the complication,and improve the quality of clinical nursing care.

ABSTRACT

Chemical investigatation of Drosera peltata var. multisepala led to the isolation of eleven compounds using various chromatographic techniques. The structures of these compounds were elucidated as isoshinanolone-4-O-beta-D-glucoside (1), isoshinanolone (2), epi-isoshinanolone (3), plumbagin (4), droserone (5), droserone-5-O-glucoside (6), quercetin (7), kaempferol (8) , gossypetin-8-O-glucoside (9), 3,3'-dimethoxy ellagic acid (10), and ellagic acid (11) by their physicochemical properties and spectral data analysis. Compound 1 was a new compound. Compounds 3, 8, 10, and 11 were isolated from this plant for the first time.

ABSTRACT

It is generally agreed that the central cholinergic system plays an important role in learning and memory processes. Alzheimers disease (AD), the most common cause of dementia, is a devastating illness characterized by progressive cognitive deterioration. The learning and cognitive deficits observed in AD patients are hypothesized to be partly caused by central cholinergic system dysfunction. There exists a malicious cycle in the pathologic course of AD, that is, the impairment of central cholinergic transmission could increase the amount of A? in the brain, and the increase of A? could also impair the efficacy of the cholinergic transmission.

ABSTRACT

AIM To study whether KB-R7943 has selective inhibitory effect on the inward and outward Na+-Ca2+ exchange current (INa-Ca) in guinea pig ventricular myocytes. METHODS Through setting up the model of intracellular Na+-overload during myocardial ischemia and reperfusion, the current-voltage relationship of INa-Ca was recorded using whole-cell patch clamp technique under bi-directional ionic conditions. RESULTS Currents were elicited by a declining ramp pulse depolarized immediately from holding potential of -40 mV to +60 mV, then repolarized to -100 mV at a speed of 80 mV*s-1 and returned to the holding potential under bi-directional ionic conditions, while the [Na+] was 25 mmol*L-1 in the pipette solution. The currents increased time-dependently and voltage-dependently which reached from (2.51±0.15) pA*pF-1 to (5.94±0.13) pA*pF-1 at +50 mV and from (-1.92±0.13) pA*pF-1 to (-3.17±0.16) pA*pF-1 at -80 mV (n=12) after 3 min and there is no significant run-down of the current. KB-R7943 10-6 mol*L-1 was found to decrease the current to (4.62±0.05) pA*pF-1 by 29.4% at +50 mV and to (-2.30±0.18) pA*pF-1 by 22.1% at -80 mV (n=5) after 5 min. While 10-5 mol*L-1 KB-R7943 was shown to decrease the current to (3.13±0.03) pA*pF-1 by 61.7% at +50 mV and to (-1.62±0.03) pA*pF-1 by 56.9% at -80 mV (n=7). CONCLUSION KB-R7943 can block INa-Ca in guinea pig ventricular myocytes. But, it did not show selective inhibition effect on inward and outward currents.

ABSTRACT

Purpose: The aim of this study is to assess the anti tumor efficacy and safety of high dose folinic acid plus 5 fluorouracil bolus and continuous infusion 48 hours combined with cisplatin in treating advanced esophageal carcinoma. Methods:Thirty patients with advanced esophageal carcinoma were treated with high dose folinic acid plus 5 fluorouracil bolus and continuous infusion 48 hours combined with cisplatin.Results: There were two complete responses, fourteen partial responses, thirteen no changes and one progressive disease in this series with total response rate of 53.33%. The main side effects include nausea and vomiting, mucositis, bone marrow suppression and alopecia. Other side effects were uncommon. All side effects were tolerable and mild except for nausea vomiting. Conclusions: High dose folinic acid plus 5 fluorouracil bolus and continuous infusion 48 hours combined with cisplatin may be a safe and effective therapy for the patients with advanced esophageal carcinoma.

The two pore domain K + Channel is a new superfamily of potassium channel subunits expressing the background potassium current on the cell membrane. Its functions are closely related with the physiological and pathological changes. This article reviews five groups of the two pore domain K + Channel on the gene and protein structure, current characteristics and pharmacological properties.
